Abstract

The wall rupture of the intestine is a life-threatening complication of ileocystoplasty and the orthotopic neobladder. We hypothesized that ischemic necrosis of the bowel in response to increased intravesical pressure could play a role in the pathomechanism of this phenomenon. In the present study, a chronic rat model was established to investigate the microcirculatory changes of the bowel and bladder wall after ileocystoplasty. A detubularised ileal segment was used for the enlargement of the bladder of the animals. 90 days after the first operation, the microcirculatory changes were observed using intravital video microscopy. The baseline values in the augmented bladder were similar to that observed in the intact organs. Microcirculation, however, was significantly diminished both in the intact bowel wall and in the intestinal graft of the ileocystoplasty, even when intraluminal pressure did not reach the leak point pressure. The microcirculation of the bladder was well preserved. Our experiments demonstrate that the increased pressure causes microcirculatory disturbances, those can play a role in the pathomechanism of the rupture of the bowel.
