Autologous buccal mucosa as a substitute for missing pendulous urethral segment was used in a 37 years old patient with urethral stricture and penile curvature after unsuccessful hypospadias surgery. For radical straightening of the penis the complete pendulous urethra had to be excised and no penile skin was available for urethroplasty. A full thickness mucosal graft measuring 2.5 x 5.0 cm was procured from the inner aspect of the cheek, tubularized with interrupted 6-zero PDS sutures anastomosed to the urethral stump and to the ventral surface of the glans. The graft-tube was fixed to the albuginea, covered by well vascularized tissue and by the skin, The patient is free of stricture or curvature 9 month after surgery, uroflowmetry: Q(max)-16 ml/s, Q(ave)-12 ml/s.
