The treatment of the ST elevation myocardial infarction has radically changed. The main role is the early reperfusion therapy mainly as a primary percutaneous coronary intervention. According to the latest guidelines the primary Pd is also indicated if the time-window is over 12 hours and the patient has a chest pain or between 12 and 24 hours without chest pain. The therapy of the cardiogenic shock has dramatically changed, there is no time and age limit. The aim is the total coronary revascularisation. New and more effective platelet inhibitors are available such as the prasugrel and the ticagrelor for the long term therapy.
