In the twentieth centrury organ transplantation became reality, where lost organ functions could be replaced. The long way to success had a lot of requirements: the safe technique of vessel anastomoses, effective immunosuppression to eliminate the immunological reactions and proper laboratory background for crossmatches. Furthermore brain death as a new entity had to be accepted to be able to transplant cadaveric organs, and the technique of conservation of these organs had to be developed. All the steps beginning from the recipient selection to the follow-up of the transplanted patient are important elements of a successful organ replacement. The first successful kidney transplantation happened in 1954 which was followed during the second half of the 1960s by pancreas and liver. Till today several houndredthousand transplantations were performed mainly in the Western world. In Hungary there is an organised renal transplant program since 1973 with four centers and about 300 transplantation per year, from 1995 we have a liver program in one center with about 45 transplantations a year and in two centers about 10 pancreas transplantations are performed per year. As a promising possibility for the future in international collaborations in Hungary successful islet transplantations were performed and there is a possibility for hepatocyte replacement also.
