Surgical radicality, i.e. the extension of the Wertheim hysterectomy following neoadjuvant chemotherapy is an unexplored area, and no randomised studies have been performed to adress this issue. The authors present the surgical strategy of their team. In their view, pelvic dissection should be tailored according to the initial, i.e. pre-treatment extension and volume of the tumour removing all tissue invaded by the tumour prior to the neoadjuvant chemotherapy.
