Delayed delivery of the second twin after miscarriage of the first fetus is uncommon. We present a case of a woman with twin pregnancy following in vitro fertilization. Expulsion of the first fetus occurred at the 20th gestational week with retention of the second twin. Following tocolytic and antibiotic treatment a cervical cerclage was performed. At the 27th week of pregnancy the second twin had to be delivered by cesarian section. Successful delivery of the newborn proved the usefulness of complex therapy in the treatment of twin pregnancy after miscarriage of the first fetus.
