In the period 1977-1980, 380 patients with recurrence of urolithiasis were studied clinically on a standard diet in the metabolic department of the University Clinic of Urology, Bonn. On the basis of the findings, the authors propose a staged program of laboratory diagnosis for determination of the etiology and for effective prophylaxis. The staged program includes a minimal program, possible in every practice, a normal program which requires a larger laboratory (main point: quantitative diagnosis on the basis of the 24-hour urine) and a special program, adjusted to the facilities in specialist departments. The special program, which includes quantitative assay of lithogenic substances in the 24-hour urine, circadian rhythm studies of the lithogenic substances and loading tests, is intended for determination of the pathogenesis as well as for the solution of special scientific problems in connection with urolithiasis.
