Abstract

The main goal in the treatment of 'spastic bladder' to the reason that kidney function was preserved and patient's complaints were released is to convert high pressure bladder into a low pressure bladder. This can be achieved reducing outflow resistance with transurethral sphincterotomy. The authors performed 'twelve o'clock' sphincterotomy in 38 'spastic bladder' at the beginning with electrocautery, later with Neodym-YAG laser. The success rate was 41% in 32 controlled patients. 14 patient needed resphincterotomy or another surgery of the lower urinary tract (TURP, TUIP, incision of the bladder neck). Among the complications partial stress incontinence was found in 4 cases. Considering that the sphincterotomy is very easy to perform and can be repeated, it seems to be appropriate treatment of 'spastic bladder', if anticholinerg medication or clean intermittent catheterisation cannot be performed.
