Phased radiofrequency ablation for atrial fibrillation recurrence after cryoballoon ablation

Abstract

BACKGROUND: Cryoballoon (CB) and phased radiofrequency (RF) ablation have been introduced into clinical practice to simplify atrial fibrillation (AF) ablation. The short- and mid-term results reported on each of these technologies are comparable to those achieved with point-by-point RF ablation. We have now evaluated the long-term success rates achieved with PVAC ablation as a redo procedure for arrhythmia recurrence after an initial CB ablation. METHODS AND RESULTS: 34 patients (9 female, age: 57, SD: 11 years) with paroxysmal (n=27) or persistent (n=7) AF who underwent phased RF ablation for atrial arrhythmia recurrence at least 3 months after an initial CB ablation were enrolled in the study. All 80 pulmonary veins, with electrical reconduction were successfully isolated acutely with phased RF ablation. During a 21.3 (SD:12) month follow-up, 27 (79%) of the 34 patients remained free of atrial arrhythmia. CONCLUSIONS: Redo ablation with phased RF offers a simple technique with high long-term success rates in patients with paroxysmal or persistent AF after a failed CB ablation.
