Abstract

Authors present their initial experience with hand assisted laparoscopic retroperitoneal Iyphadenectomy for nonseminomatous testicular cancer. Six patients were operated on, four with right side, two with left side tumor. Three patients had clininal stage I, two stage II/A and one stage II/B. Depending on the tumor's site a 7 cm mid lower or upper laparotomy was done and through a " hand-port" the surgeon's left hand was inserted into the abdomen and 4 laparoscopic ports were used. The colon ascendens or descendens was mobilized completely. The retroperitoneal lymphatic tissue were dissected and removed according to the modification of Weissbach. The average operative time was 4,9 (2,5-7,2) hours. The postoperative period was uneventful, the patients were discharged from the hospital on the 3rd postoperative day. All patients exept the one with pathological stage I received 2-3 courses of combined chemotherapy. The hand assistance makes the laparoscopic operation simpler and safer and the 7 cm muscle splitting incision is still fairly advantageous for the patients compared to the xyphopubic laparotomy of the open procedure. To our knowledge this is the first report on hand assisted laparoscopic retroperitoneal lymph node dissection.
