Abstract

Under physiologic circumstances the function of the cardiovascular system is based upon an intact endothelium. The different processes related to the endothelium sustain a steady-state between the constrictor and vasodilator, procoagulant and anticoagulant mediators, inflammatory and anti-inflammatory, growth-inhibiting and growth-stimulating substances. Endothelial dysfunction is considered to be a disturbance of this physiological steady state. It is the first step in the pathophysiology of atherosclerotic cardiovascular diseases, which precedes morphological changes and indicates disturbances of normal oxido-reduction processes of the cells. Beside description of some endothelium produced substances and presentation of their role in endothelial dysfunction, the clinical ascpects and examination of dysfunctious endothelium are shown. After description of the provocation tests (Ach, FMV) this paper presents patients with angina pectoris and normal coronarography where the confirmation of endothelial dysfunction is an important task.
