Abstract

A short review of literature in connection with equine pulmonary fibrosis is given and a clinical case in a twelve-year-old gelding is described. Exercise intolerance, chronic fever and dyspnea appeared in the anamnesis. Main clinical signs were the followings: increased body temperature, heart rate and respiratory rate, slight serous bilateral nasal discharge, dyspnea and abnormal respiratory sounds. Pulmonary interstitial infiltrates, increased lung opacity and a diffuse bronchial and interstitial pattern could be observed by thoracic radiography. The bronchoscopic findings showed a little amount of greenish white thick discharge in the trachea and a medium degree edema of the carina. The thoracic ultrasonography of the 10th and 11th intercostal spaces near the caudal lung border and the echocardiographic examination showed no abnormalities. Ultrasound guided percutaneous lung biopsy was performed with an automated biopsy instrument through the left and right 10th intercostal space in the medium third of the thorax, but the biopsy specimen was of no diagnostic value. The laboratory examination of the blood revealed a relative neutrophil granulocytosis. The AST-, alkalic phosphatase-, lipase- and LDH-activities of the plasma were increased, and the total protein level was decreased. The horse did not respond to intensive treatment, therefore it was euthanised. The pathologic and hystopathologic examinations showed severe diffuse interstitial pneumonia with fibrous connective tissue proliferation (lung fibrosis).
