Abstract

Cryopreservation of human oocytes has been integrated into the repertoire of the assisted reproductive technologies (ART), heralding a significant treatment option for women. Its application provides fertility preservation to counter declining ovarian reserve with age, opportunity to purposefully delay reproduction, allow allocation of oocytes for freezing and storage rather than insemination during a treatment cycle to forestall creation of supernumerary embryos, and catalyzes the establishment of donor oocyte banks with their provision of wider variety in donor types, greater convenience in time and logistics, and reduced expense to the recipient. A review of the effort to achieve a method for cryopreserving and thawing human oocytes that yields high recovery, survival, fertilization, embryo development, and implantation rates is provided in conjunction with a model for creating a donor oocyte bank. The implications of this most recent of assisted reproductive techniques are considered in the context of the projected expectations of future generations of patients seeking ART to build their families.
