In the last two decades, there has been an increasing interest in Europe in treating selected patients with early-stage breast cancer with accelerated partial breast irradiation (APBI) using interstitial or intracavitary (MammoSite) brachytherapy. In this chapter, we give an overview of these European clinical trials of APBI, including their implications for optimal patient selection, target definition, treatment technique, and quality assurance. Finally, we will discuss the development and status of the new European multicentric Phase III APBI trial conducted by the Breast Cancer Working Group of the Groupe Européen de Curiethérapie-European Society for Therapeutic Radiology and Oncology.
