A new instrument (semitrepan) has been designed in order to decrease the severe postoperative astigmatism following keratoplasty. Using the semitrepan two incisions are made to a depth of 0.3 mm in the transplantation line at both ends of the meridian of higher refraction. The operation can be carried out on an outpatient basis. In one case the patient's astigmatism decreased from 18.0 D to 3.0 D.
